Moles/ Naevi/ Birthmarks/ Tattoos

Mole and naevi are a localized aggregation of naevus cells. They often appear all over our body during the first 2 decades. They are often round or oval in shape, the colour varies from black to brown to a deep blue. The body may have up to a hundred moles in quantity. They are mostly benign.

Some moles are quite shallow in terms of skin depth, we call them junctional naevus; some are located deeper within the skin, and we call them compound naevus. For those that protrude 5mm above the skin surface, we call them intradermal naevus.

We often want to remove moles for 2 reasons. The first being cosmetic, and the second being a belief in destiny. This is especially common amongst the Chinese, who believe that a particular mole on the face may have an impact on their destiny. Carbon dioxide laser is used to ablate the mole, and in most cases, no scar is seen. For suspicious looking mole, we may need to surgically remove for histopathology report. Some beauty salons use corrosive acid to burn off the mole. This often leads to permanent unnecessary scarring.

A Birth mark is a congenital area of pigmentary skin change, it can easily be removed by surgery.

Intense Pulsed Light (IPL)
Our eyes can only see the rainbow colours, ranging from a wavelength of 400nm-700nm. Intense pulsed light is a coherent light source ranging from 500nm-1000nm. When IPL is shot through the skin, it can target the pigments (melanin and haemoglobin) and thermally destroy them. It can also stimulate the dermal collagen and restore skin elasticity. As a result, it can rejuvenate the skin due to ultraviolet radiation damage, and effectively reduces skin pigment and unsightly blood vessels.

IPL is ideal for freckles and solar lentigines, and can reduce facial redness such as portwine stain and rosacea. You will need 5 monthly treatments, and maintenance treatment every 6 months to keep your skin vibrant.

Carbon dioxide laser
Carbon dioxide laser uses thermal energy to ablate the skin in very thin layers, and is the tool of choice to remove moles, seborrhoeic keratoses and syringomata.

It will leave an ablated wound which will form a scab in a few days. When the scab falls off, the skin will appear slightly pinkish, and will return to normal appearance in about 2-3 months.

Q-switched Nd:YAG laser
Q-switched laser will create a photoacoustic effect, and is very effective in shattering deep-seated pigments such as melasma and tattoo. Our macrophages will then come and gobble up these shattered pigments.

Many people tattoo themselves when they were young. When they grow older, they often regret the decision. Q-switched laser can remove tattoo very effectively without leaving any scars on the skin. It can also remove deep dermal pigments such as Naevus of Ota, Hori’s macules, Café au lait macules and even melasma. You may need more than 1 treatment before you see them disappear completely.

Aramis 1540nm laser
This infra-red laser will penetrate the skin and cause microtrauma to the dermis. As a result it will trigger the collagen to reproduce and give our skin more elasticity. It will help to reduce the unevenness of acne scars. The handpiece of the laser has contact cooling, so you will not find any discomfort at all, unlike similar infra-red lasers such as the Cooltouch CT3 Plus.

We often pay too much attention to our skin surface, and neglect the elasticity part. A treatment course of Aramis laser will need 5 monthly treatments. You will find your skin with far less fine wrinkles than before.

Erbium-YAG laser
This is another ablative laser that thermally removes a thin layer of skin. Unlike the carbon dioxide laser, we use this laser to resurface the whole face. This is not a laser for everybody, but if you can go through an unsightly 2 weeks downtime, it can improve photoaging, wrinkling, and sagging dramatically. Personally, I reserve this laser for those who cannot get good results from other forms of treatment.

Gentle Wave LED
Gentle Wave LED employs photomodulation. By flashing a 590nm yellow LED light in a particular manner, it can stimulate the skin to produce more collagen. The treatment is performed twice weekly, and will require 8 sessions. The treatment is simple and slick, you will not feel any discomfort at all. When supplemented with other treatment, it has a synergistic effect and will enhance the overall result.
